In a heartbreaking incident in Itsoseng, Hebron, North West, eight-year-old Nhlanhla Monyai lost his life on Monday, 22 September 2025, after a large concrete pipe rolled over him near the Burkina Faso River.
The pipe, reportedly left unsecured in a sloping area, was being played with by Nhlanhla and two other children when the accident occurred.
Victor Ngulele (43), Nhlanhla’s uncle, expressed the family’s grief and frustration, alleging negligence by local authorities. “We want justice for our little kid. Why was the pipe left exposed where children could access it?” he questioned. The family is demanding accountability from Ward 16 councillor John Makhubela, seeking answers about why the pipe was abandoned.
Nhlanhla’s father, Lebo Mokoena (36), described his son as a kind and special soul. “If that pipe wasn’t there, my son would still be alive,” he said, devastated by the loss. The family is calling for an apology from the councillor and action to prevent similar tragedies.
Councillor Makhubela explained that the pipe was one of three requested by the community in 2022 from a local mall. While two pipes had been used, the third remained unused due to delays from the Madibeng Local Municipality in assisting with its placement. “This is a tragic and unfortunate incident.
I’m deeply disturbed, as is the community,” Makhubela said, extending condolences to the family. He noted that community elders had previously tried to stop children from playing with the pipe and called the incident a wake-up call for the municipality to address urgent community requests.
North West Police spokesman Brigadier Sabata Mokgwabone confirmed that an inquest docket has been opened. Nhlanhla succumbed to his injuries at Odi Hospital, and investigations are ongoing to determine the circumstances of the accident.
Nhlanhla was laid to rest at Itsoseng Cemetery on Sunday, 28 September 2025. The tragedy has left the community shaken, highlighting the dangers of unsecured infrastructure and the need for swift municipal action to ensure public safety.
